Publishing a Form on Your Shopify Store

Once you have created and published your registration form in Rivio B2B Registration Forms, you can add it to your Shopify storefront. This allows customers to access the form and submit their applications directly from your store.

Step 1: Open the Shopify Theme Editor

1. Log in to your Shopify Admin.
2. Go to Online Store → Themes.
3. Click Customize on your active theme to open the Shopify Theme Editor.
4. Navigate to the page where you want to display your registration form.

Step 2: Add the Wholesale Form Block

In the Shopify Theme Editor:

1. Select the page where you want to display the form.
2. Click Add section or Add block, depending on your theme.
3. Select Apps.
4. Choose Wholesale form.

The Wholesale Form block will be added to your selected page.

Step 3: Enter Your Form Slug

After adding the Wholesale Form block, its settings will appear in the right-hand panel.

Locate the Form slug field and enter the slug of your published form.

You can find the Form Slug in Rivio B2B Registration Forms:

Forms → Select your form → Details

For example:

wholesale-application

Enter the Form Slug exactly as it appears in your form's Details section.

Step 4: Preview Your Form

After entering the Form Slug, check the storefront preview in the Theme Editor.

Make sure the correct registration form is displayed and that the fields appear as expected.

If the wrong form appears, check that you entered the correct Form Slug.

Step 5: Save Your Changes

Once everything looks correct, click Save in the Shopify Theme Editor.

Your registration form is now published on the selected page of your Shopify store and customers can use it to submit applications.

Important: Form Slug

The Form Slug tells the Wholesale Form block which registration form to display.

For example, if your published form has the slug:

wholesale-application

Enter the same value in the Form slug field in the Shopify Theme Editor.

Tip: Always copy the Form Slug directly from your form's Details section to avoid spelling mistakes or displaying the wrong form.

Example

Suppose you created a form called Wholesale Application Form and its Form Slug is:

wholesale-application

You can add the Wholesale form block to your Contact, Wholesale, or any other suitable page, enter wholesale-application in the Form slug field, and save the changes.

Customers visiting that page will then see your wholesale registration form and can submit their applications.
