Rivio B2B Registration Forms lets you start with ready-to-use templates and customize them to match your business needs. You can choose a template, update its fields, and create a registration form that collects the information you need from your customers.

Step 1: Open Templates

In your Shopify Admin, open Rivio B2B Registration Forms and go to Templates.

You'll see a variety of ready-to-use templates for different purposes, such as wholesale applications, retailer applications, distributor applications, contact forms, feedback forms, and trade professional applications.

Step 2: Choose a Template

Review the available templates and choose the one that best matches your requirements.

Each template shows:

  • A short description of the template.
  • The fields included in the template.
  • The number of required fields.
  • Whether file uploads are included.
  • Tags that can be applied when an application is approved.
  • Whether the template can create a B2B company on approval.

Click Use template to start creating your form.

Step 3: Review the Form Fields

After selecting a template, review the fields included in the form. Templates provide a starting point, so you can adjust the form based on the information you want to collect.

You can add new fields, edit existing fields, remove fields you don't need, and change the order of fields.

Step 4: Customize Your Form

Customize the form to suit your business requirements. You can update field labels, make fields required or optional, and configure the available field settings.

For example, a wholesale registration form may collect business details, primary contact information, resale certificates, tax information, and estimated purchase volume.

Step 5: Review the Form

Before publishing the form, review all fields and settings to make sure the form collects the information you need.

Check that:

  • All required fields are included.
  • Optional fields are configured correctly.
  • Field labels are clear and easy for customers to understand.
  • File upload fields are included when supporting documents are required.
  • The form is configured for your approval workflow.

Step 6: Save and Publish

Once your customization is complete, save your changes and publish the form.

Your customized form can then be added to your Shopify store using the Wholesale Form block in the Shopify Theme Editor.

Why Customize a Template?

Templates help you create a form quickly without starting from scratch. Customizing the template allows you to collect only the information that is relevant to your business and your customers.

Tip: Start with the template that is closest to your requirements, then customize the fields instead of building the entire form from scratch.
