Rivio B2B Registration Forms helps Shopify merchants create and manage registration forms for wholesale customers, retailers, trade professionals, and other business applications.
You can create a form, add it to your Shopify storefront, receive applications, review submissions, and approve or reject applicants — all from your Shopify Admin.
How Rivio B2B Registration Forms Works
The app follows a simple process:
Create Form → Publish Form → Display on Store → Receive Applications → Review Submissions → Approve or Reject
Step 1: Create Your Registration Form
Start by creating a new registration form from the Forms section.
You can either:
- Choose a ready-to-use template.
- Create a blank form from scratch.
Templates are available for different business needs, such as:
- Wholesale Registration
- Retailer / Stockist Application
- Trade Professional Program
- Quote Request
- Newsletter Signup
- Contact Form
- Feedback Form
- Short Survey
You can customize the selected template by adding, removing, or editing fields.
Step 2: Customize Your Form
Once you've selected a template, customize the form according to your business requirements.
You can:
- Add new fields.
- Remove fields you don't need.
- Edit field labels.
- Reorder fields.
- Make fields required.
- Add descriptions or helper text.
- Configure form settings.
This allows you to collect exactly the information you need from your customers or wholesale applicants.
Step 3: Publish Your Form
After customizing your form, save and publish it.
Once published, your form will have a Form Slug that identifies the form when you add it to your Shopify storefront.
For example:

You can find the Form Slug in your form's Details section.
Step 4: Add the Form to Your Store
After publishing the form, add it to a page on your Shopify store using the Wholesale Form app block.
From the Shopify Theme Editor:
- Open the page where you want to display the form.
- Click Add Section.
- Select Apps.
- Choose Wholesale Form.
- Enter the Form Slug of your published form.
- Click Save.
Your registration form is now available on your storefront.
Step 5: Customers Submit Applications
Customers can visit the page containing your registration form and submit their information.
Depending on the form you've created, customers may provide details such as:
- Business name
- Contact information
- Business address
- Website
- Tax ID / VAT
- Business type
- Purchase volume
- Supporting documents
- Additional information
Once the form is submitted, the application appears in the Submissions section of the app.
Step 6: Review Applications
Go to Submissions to view applications received through your registration forms.
You can:
- Search for submissions.
- Filter submissions by form.
- Filter submissions by status.
- Open an application to view its details.
- Review uploaded files.
- Add internal notes.
This gives you all the information you need to evaluate an application.
Step 7: Approve or Reject Applications
After reviewing an application, you can decide how to proceed.
Approve
If the applicant meets your requirements, click Approve.
The application status will change to Approved.
Depending on your configuration, approval may also trigger configured actions such as customer tagging or B2B company creation.
Reject
If the applicant does not meet your requirements, enter a reason in the Reason (sent to applicant) field and click Reject.
The application will be marked as Rejected.
Mark as Spam
If a submission is unwanted or irrelevant, you can use Mark as spam to keep your submissions organized.
Step 8: Manage Your Applications
You can easily track applications using the submission statuses:
- Pending – Applications waiting for review.
- Approved – Applications that have been approved.
- Rejected – Applications that have been rejected.
- Spam – Unwanted or irrelevant submissions.
Simple App Workflow
The complete process can be summarized as:
- Create a Form – Choose a template or create a form from scratch.
- Customize – Add and configure the fields you need.
- Publish – Make your form available for use.
- Display – Add the Wholesale Form block to your Shopify store.
- Collect Applications – Customers submit their information.
- Review – Check the submitted details and documents.
- Take Action – Approve, reject, or mark the submission as spam.
- Manage – Track applications using submission statuses.
