How to display a form on your website?

Display Your Registration Form on Your Store

Once you've created and published your registration form, you can add it to any page in your Shopify store so customers can submit wholesale applications.

Step 1: Open the Shopify Theme Editor

1. In your Shopify Admin, go to Online Store > Themes.
2. Click Customize on your active theme to open the Theme Editor.

Step 2: Add the Wholesale Form Block

1. Navigate to the page where you want to display the registration form.
2. Click Add Section.
3. Select Apps.
4. Choose Wholesale Form.

The Wholesale Form block will be added to the selected page.

Step 3: Select Your Form

1. In the Wholesale Form block settings, locate the Form Slug field.
2. Copy the slug from your published form by navigating to:

• Rivio B2B Registration Forms > Forms
• Open your published form.
• Copy the Form Slug from the Details section (for example, wholesale-application).

3. Paste the slug into the Form Slug field in the Theme Editor.

Step 4: Save Your Changes

1. Click Save in the Theme Editor.

Your wholesale registration form is now live and ready for customers to submit applications.

Tip

The Form Slug tells the Wholesale Form block which registration form to display. Make sure you copy the slug exactly as shown in the Details section of your form.
