Rivio B2B Registration Forms makes it easy to display your registration forms on different pages of your Shopify store. You can add a form to a dedicated registration page, contact page, wholesale page, or any other supported page where you want customers to submit their information.

Before You Begin

Before adding a form to a page, make sure:

  • Rivio B2B Registration Forms is installed.
  • You have created the required form.
  • The form is published.
  • You have access to the Shopify Theme Editor.

Create or Select a Shopify Page

First, decide where you want to display the form.

For example, you can create pages such as:

  • Wholesale Registration
  • Become a Retailer
  • Distributor Application
  • Trade Registration
  • Business Registration

If the page already exists, you can use the existing page.

Open the Shopify Theme Editor

To add the form:

  1. Open Shopify Admin.
  2. Go to Online Store → Themes.
  3. Find the theme you want to edit.
  4. Click Customize.
  5. Use the page selector at the top of the Theme Editor.
  6. Select the page where you want to display the form.

Add the Form

Once you have opened the required page:

  1. Click Add section or Add block.
  2. Select Apps.
  3. Find Rivio B2B Registration Forms.
  4. Add the form block to the page.
  5. Select the published form you want to display.

The available placement options may vary depending on your Shopify theme.

Select the Correct Form

If you have created multiple forms, make sure you select the correct one.

For example:

  • Use a Wholesale Application form on your wholesale page.
  • Use a Distributor Application form on your distributor page.
  • Use a Retailer / Stockist Application form on your retailer page.

This helps ensure customers see the appropriate form for their requirements.

Save and Preview the Page

After adding the form:

  1. Review the form in the Theme Editor.
  2. Check that the form appears in the correct location.
  3. Make sure all fields are displayed properly.
  4. Click Save.
  5. Open the page on your storefront.

Submit a test application to make sure the form is working correctly.

  1. Make sure the form is published.
  2. Confirm that the correct theme is being edited.
  3. Refresh the Shopify Theme Editor.
  4. Check that the Rivio B2B Registration Forms app block has been added.
  5. Make sure the app block is placed in a supported section.
  6. Save the theme changes and check the storefront again.

If the app block is not available, check whether the app's Theme App Extension is enabled.

Remove a Form From a Page

If you no longer want to display a form on a particular page:

  1. Open Shopify Admin → Online Store → Themes.
  2. Click Customize.
  3. Open the page containing the form.
